'Captains' Day at States' forms due Monday, Nov. 16

The FHSAA is continuing “Volleyball Captains’ Day at States” at the FHSAA Girls Volleyball Finals.  We are going to recognize the varsity volleyball team captains from all FHSAA member schools that are in attendance for the first match on Saturday, November 21.  Your captain(s) will be:

• recognized by name on the FHSAA web site
• recognized by name in the 2009 tournament program
• recognized with photo in the 2010 tournament program
• recognized by name on the message board at The Lakeland Center
• Introduced to the crowd on the arena floor (prior to or between games) during the first match of the day.  This match begins at 12 p.m.
• presented a commemorative certificate suitable for framing

Each captain is responsible for securing their own transportation and for purchasing her ticket.  A special reserved seating section will be provided in the George Jenkins Arena.  Your captain will be given a complimentary copy of the tournament program.  After the awards ceremony following the match, captains will be provided a light meal in a hospitality room reserved for these outstanding student-athletes.  A group photo will be taken of those in attendance.

The enclosed form needs to be faxed to the FHSAA Office (352.244.5052) no later than 4 p.m. on Monday, November 16, 2009.  Please understand, this is a commitment to attend.
